This template is based on a '5-Whys' session to enable a team identify root causes of a problem. The first step is a brief brainstorming discussion to get a complete and useful problem description. Then, iteratively, the team:
Propose a 'Why' question to delve into the problem
Choose a cause within a set of plausible answers
Iterates (desirably up to 5 times) to discover the root cause.
